It’s hard to believe that it’s already time to start thinking about scheduling your child’s sports physical exam. Didn’t summer just begin? In a blink of an eye, it will be over and school will be starting. We want our children healthy and ready to participate in school activities. Community Health Centers provide sports physical days on several dates and in several locations. No need to miss out!

Riverton Community Health: Walk-ins only. August 4, 5 and 6 from 8:00 am – 11:00 am and 1:00 – 4:00 pm (we will be in our new location on August 3rd at 1035 Rose Ln)
Insurance cards and photo ID are required. We will also need pertinent information from the insurance policyholder for billing purposes and the parent/guardian will need to sign required consent and registration forms. Parent or guardian (if under 18) must be present for Sports Physicals to be completed. Students (under 18) without parent/guardian will not have their physicals completed. We will bill insurance/Medicaid, but we do not bill for co-pays or deductibles.
