We want our patients to know Community Health Center of Central Wyoming (CHCCW) is dedicated to the health and well-being of you and your family. The purpose of this notification is to inform you effective October 4, 2019, Dr. Jerry Jarrard, MD, is no longer practicing at CHCCW.
We are taking diligent steps to transition your care to the provider of your choice within our clinic or we can work with you to obtain your records for establishing care outside of CHCCW.
• Hollie Johnson, FNP-C – Primary Care is currently taking new patients.
• Sandee Lewis, DNP – Primary Care is currently taking new patients.
With new providers there is no guarantee the previous prescribing practices or treatments will be continued, as each provider tailors their treatment of patients based on their own scope and style of practice. Establishing care with a new provider within our practice will be required to continue services within our organization.
We wish Dr. Jarrard the best in his future endeavors and appreciate the phenomenal care he has provided our patients over the last five years.
